2009 Commencement Information

Event Date: Thursday, May 7, 2009
Time: Event begins 2:00 p.m.; Student lineup begins 1:00 p.m.
Location: UIC Forum, 725 W. Roosevelt

Where to Assemble
Candidates should enter through the main entrance on 725 W. Roosevelt. You will be directed to student line up by a Forum staff person. Students will assemble by department and program at 1:00 p.m. Faculty marshals will have signs indicating where students from each program should line up.

Tickets
Tickets will not be required for admission to the AHS Commencement ceremony. Graduates are welcome to invite as many guests as they would like. Seating will be on a 'first-come, first-serve' basis. Doors will open at 1:00 p.m.

RSVP
It is important that graduates notify the College whether they are planning to attend the AHS Commencement ceremony by April 30. Graduates should RSVP online and complete an exit survey. Graduates can also notify their program's Commencement Coordinator (see list below). Please respond as this is critical for planning purposes.

Reception
A reception will immediately follow the AHS Commencement ceremony in meeting rooms D, E, and F on the first floor of the Forum. Light refreshments will be served.

Caps & Gowns
Please note that academic dress is required for the College Commencement ceremony. Undergraduate student caps and gowns may be purchased from the bookstores in the UIC Student Centers East (312-413-5500) or West (312-413-5500). Graduate and professional students should purchase their attire in the Medical Bookstore in the UIC Student Center West.

The cost, excluding sales tax, is:

  • $28.95 for bachelor's attire
  • $54.95 for master's attire
  • $82.95 for PhD attire
  • $62.95 for DPT and OTD attire

Phone orders will be accepted beginning April 1 and gowns will be on sale in the bookstores beginning April 6.

Tassels

  • Bachelor of Science students will wear a gold tassel.
  • Bachelor of Arts students will wear a white tassel.
  • Master of Science students will wear a green/gold tassel and gold-lined hood.
  • Ph.D. students will wear a gold tassel and a hood unique to their discipline.
  • Doctor of Physical Therapy students will wear a teal tassel and a teal-lined hood.
  • Occupational Therapy Doctorate students will wear a rhodes blue tassel and a rhodes blue lined hood.

A tassel is included with cap & gown orders, but additional tassels are available in the campus bookstores.

University Honors / Cords
University Honors are awarded to undergraduate students who are earning a degree and whose cumulative grade point average falls within the following honors categories: Summa cum laude: 3.90 and above; Magna cum laude: 3.75 to 3.89; or Cum laude: 3.50 to 3.74. Transfer students must have completed a minimum of 42 semester hours as of end of Fall term prior to graduation and a minimum of 60 hours by the time of graduation at UIC to be eligible. Students eligible for University Honors will receive an email notification from the Office of the Vice Provost for Undergraduate Affairs. If you believe you are eligible for University Honors and have not received a letter of notification by April 18, please contact Elaine Nicholas at (312) 996-2077. Students eligible for honors will receive a cord that should be worn during the ceremony. Honor's cords will be distributed at the Honors College Convocation and at the Grad Finale Ceremony or by arrangement with your department's Commencement Coordinator.

Accessibility Accommodations
Provisions will be made for graduates with disabilities, if needed. Please make your program coordinator aware of any accommodations required. Special needs/hearing-impaired guests may obtain seating assistance by going to the Information Desk directly across from the main entrance doors. Drop off for wheelchair users is at the main doors of the UIC Forum on Halsted Street. At 718 W. Rochford Street there is a walkway that leads to the UIC Forum's main entrance. Cars can drop off wheelchair users there. A sign language interpreter will be present on the stage for the benefit of students and guests who are hearing impaired. If you have additional questions or concerns, please contact Elaine Nicholas, AHS Commencement Coordinator, at 312-996-2077.

Diplomas and Diploma Covers
It is recommended that you access your final grade report via the Web for Student System for your records. Your name as it appears in the Web for Student system as of the last day of classes will appear on your diploma. If you have a change of name that you would like to appear on your diploma, please complete the Current Student Diploma Name Change form to request changes to your name. If you should have any questions, please contact the Records Office at 312-996-4381, M-F, 8:30 am to 5:00 pm.

Diplomas will be mailed two to three months after graduation. The printed diploma will be delivered to your active mailing address, unless you have created an active diploma address in the Web for Student system. Addresses can be updated from the Personal Information menu. Any changes to the delivery address of the diploma after the last day of classes should be reported to the Records Office at 312-996-4381, M-F, 8:30 am to 5:00 pm.

Diploma cover: Exit Survey Required
Diploma covers will be provided at the Commencement ceremony as a gift from the University of Illinois Alumni Association. All graduates are required to complete the Alumni Association’s online exit survey on the UIAA website.

Photography Services
Photography services will be provided by Chappell Graduation Images. An individual photograph will be taken of each graduate as they receive their diploma cover. A photo proof will be sent to each graduate's mailing address approximately one week after the ceremony. Prints are typically available two weeks after an order has been placed. Please be sure to update your mailing address in the "Web for Student" system to ensure proper delivery.

Video
Copies of the ceremony broadcast will be available on DVD. Details on ordering a video copy of the ceremony are available on the UIC Commencement website.

Parking
Parking will be available for $5.00 in the UIC Parking Lot 5C located at 1135 South Morgan Street and Lot 6 (six) Halsted and Roosevelt. Students with current UIC parking assignments can show their hangtags to waive this $5.00 fee. Visitors can also park at the Maxwell Street Parking Structure located on 701 W. Maxwell Street. Parkers can enter from Maxwell Street or from Union (one way street). Refer to the UIC Commencement website for additional details and maps for parking.
