Admission Criteria | Application Materials

A complete application to the fellowship program must include the following materials:

• Curriculum vitae and cover letter: The cover letter should address how your past experience has prepared you for this type of educational program.
• Two Letters of Recommendation: Please submit recommendations from physical therapists with direct knowledge of your clinical knowledge and skills.
• Transcripts: Applicants must provide official copies of all college and university transcripts.
• Application fee: There is a nonrefundable application processing fee of $25, payable to UIC Department of Physical Therapy. This fee is waived for current employees of the UIC Physical Therapy Department.
