How to Apply - PhD Program

APPLICATION MATERIALS

A complete application to the PhD in Disability Studies program must include the following:

  • UIC Graduate Application: Complete the Graduate College Application online.
  • Application fee: Submit a nonrefundable fee of $50 for domestic applicants, $60 for international applicants. Checks should be made out to “UIC.”
  • Official transcripts: Applicants must request transcripts from each university and college attended. Transcripts should be sealed in an envelope by the Registrar and submitted, unopened, with your application package.
  • GRE scores: Scores should be sent directly from Education Testing Service to UIC. If you have taken the GRE within the past five years, you must request that scores be sent from ETS to UIC. UIC Institution Code: R1851
  • Statement of Academic Interests: Each applicant is required to submit a 300-500 word Statement of Academic Interests. This statement should describe your research interests and focus, including a discussion of how the area of disability studies and the research focus of this program are of interest to you. This statement should be typewritten and double spaced. 
  • CV: Please include a current CV.
  • Three letters of recommendation: Persons who can comment meaningfully on your work in academic and professional settings are best suited to submit letters of recommendation. Do not seek recommendations from peers, family, friends, or social acquaintances. Download the recommendation form and send it along with a self-addressed envelope marked “Letter of Recommendation” to three people. Each person should be asked to complete the form and return it to you in the self-addressed envelope with his or her signature affixed over the seal.
  • Graduate Appointments Form (optional): For students seeking a research assistantship or traineeship, download and submit the Application for Graduate Appointments.
  • International applicants only: Applicants must submit TOEFL scores and a Declaration and Certification of Finances form (must be accompanied by an original or certified copy of a bank statement showing proof of sufficient funds in U.S. dollars).   • Application checklist: Download and complete the application checklist for and submit it with your application.
