- Official transcripts: Applicants must request transcripts from each university and college attended. Transcripts should be sealed in an envelope by the Registrar and submitted, unopened, with your application package.
- GRE scores: Scores should be sent directly from Education Testing Service to UIC. If you have taken the GRE within the past five years, you must request that scores be sent from ETS to UIC. UIC Institution Code: R1851
- Disability and Human Development Application: Complete the department's application.
- Curriculum Vita
- Personal statement: Enclose a 350 - 500 word statement addressing your goals for graduate study and career development. The purpose of the personal statement is to allow you to tell us why you should be accepted for graduate study in Disability and Human Development. A personal statement should state why you are interested in this area of study, include a discussion of your career goals, and describe notable life or work experiences that have contributed to your pursuing graduate work in Disability and Human Development. Please feel free to expand upon any of the life, work or volunteer experiences that you have included on your Disability and Human Development Application Form. You should take great care in preparing your personal statement. A thoughtful and well-written personal statement is a good investment of time and effort.
- Three letters of recommendation: Persons who can comment meaningfully on your work in academic and professional settings are best suited to submit letters of recommendation. Do not seek recommendations from peers, family, friends, or social acquaintances. Download the recommendation form and send it along with a self-addressed envelope marked “Letter of Recommendation” to three people. Each person should be asked to complete the form and return it to you in the self-addressed envelope with his or her signature affixed over the seal.
- Final checklist: Please complete the Application Submission Checklist. Check your application package against the checklist to insure that all materials are included. You are responsible for submitting all the materials. Incomplete applications will not be considered. Official GRE or TOEFL scores MUST be sent directly from Educational Testing Service.
- International applicants only: Applicants must submit TOEFL scores (UIC Institution Code 1851) and a Declaration and Certification of Finances form (must be accompanied by an original or certified copy of a bank statement showing proof of sufficient funds in U.S. dollars).
