How to Apply - Health Information Management, BS
Students are encouraged to apply early in the fall of the year prior to completing the 60 semester hours of prerequisite courses. Applications may be submitted prior to completion of the required hours as long as the student completes them prior to starting the program.
Application materials must be complete and postmarked by March 31 in the year of intended enrollment.

- UIC Undergraduate Application – Transfer application form [pdf] or a printed copy of the completed online application. Applicants are strongly encouraged to apply online for faster processing. The major code for Health Information Management is 20GF1304BS.
- Application Fee – A nonrefundable application fee ($40 domestic/immigrant; $50 international) is required. Payment may be made by sending a check or money order in US currency payable to the University of Illinois or, if applying on-line, by paying with MasterCard or Visa.
- Transcripts – An official transcript mailed directly from each college or university you have attended must be submitted. Transcripts marked 'Issued to student' are considered official only if they are received in a sealed envelope from that school.
- Information Form [pdf] - Please fill out completely.
- Course Outline Form [pdf] - Next to each course indicate where the course was completed with the term and year and the grade received, as well as courses in progress
- Three Letters of Recommendation [pdf] - Three letters of recommendation are required (print 3 copies) and may be obtained from academic advisors, college professors, employers or others familiar with your past academic achievements or work history. Letters from friends or family members are not accepted.
International and Immigrant/Other Noncitizen Students only – In addition to all of the above, you will need to submit a TOEFL score of 550 or higher, Evidence of Financial Support and Immigration Status, and your ECE Special Report (foreign coursework). See Instructions [pdf] for more information.
Intercollege Transfer Students only – Undergraduate students currently enrolled at UIC, or who have been accepted for spring or summer semesters of the current year, are considered intercollege transfer students and need only submit:
The Undergraduate Application is not required and no fee should be paid. Additional transcripts must be submitted only if you have attended other institutions since enrolling at UIC.
